Personal Support Staff

Duties and Responsibilities:
- Reviews and follows each person’s Coordinated Service and Support Plan (CSSP) and set Activities of Daily Living (ADL’s)
- Provides assistance with hygiene care as directed, including but not limited to: bathing, grooming, oral care, dressing, undressing, toileting activities, etc.
- Provides assistance with daily tasks, including but not limited to: feeding, meal preparation, laundry, housekeeping, providing transportation and support to doctor appointments, leisure and outdoor activities, and community involvement and integration.
- Attends Employee Trainings, Meetings and Workshops as required.
- Meets all state/federal rules and regulations that apply, while also maintaining required knowledge and understanding of persons served Individual Abuse Prevention Plan and Self-Management Assessment.
- Documents service delivery in accordance with Department of Human Services Regulations.
- Completes Safe Medication Assistance and Administration per Policy and Procedure requirements.
- Assists persons served with money management while following all financial policies and procedures.
- Assists individuals and their support teams in providing opportunities for the individuals’ choices and present creative options as needed.
- Displays knowledge and understanding of persons served diagnoses and all health care needs.
- Portrays a positive role as advocate for each individual person served at all times.
- Ensures all of person served medical needs are addressed and met.
- Follows and displays knowledge and understanding of all routines and guidelines.
- Observes and reports any substantial findings and/or changes in the person served behavior to the supervisor while documenting the findings when necessary.
- Observes and reports all injuries and incidents per requirements.
- Performs additional assigned duties at the direction of supervisors.
MINIMUM REQUIREMENTS
- High school diploma or GED preferred.
- Must pass a Department of Human Services background check.
- Must have a valid Minnesota Driver’s License, preferred for most positions.
- Must have and maintain insurance on a personal vehicle as required by Minnesota State Law.
- Must be able to transport persons served locally and out-of-town as needed.
- Successfully show reliability and competency in each of the Personal Support Staff domains.
- Must be able to verbally and physically intervene or redirect person served as necessary.
- Must be able to fluently read, write and speak in the English language.
- Must be able to communicate effectively and efficiently.

PHYSICAL REQUIREMENTS:
Ability to perform the following tasks if necessary:
- Ability to participate in physical activity.
- Ability to work for extended period, while standing and being involved in physical activity.
- Moderate to heavy lifting
- Must be able to lift at least 30 pounds and transfer/lift person served as needed.
- Ability to do extensive bending, lifting, and standing on a regular basis.
POSITION DETAILS
This position reports to the Personal Support Manager (PSM). The hours expected in this position will vary by person served and business needs.
There is a 90-day probationary period for this position. We encourage regular communication with your supervisor during this period to ensure you fully understand these expectations and to solicit the feedback necessary to be successful. You may expect to receive a formal review following the probationary period and then on an annual basis going forward.
SummaryAs a Personal Support Staff, you will perform personal care activities of daily living and other assigned functions of personal care for each individual person ICAN serves. This position is responsible for creating a quality and enriching living experience by meeting the goals and care plans set forth for each person served. This position serves and acts as an advocate, teacher, and mentor for each individual.
